401(k) Enrollment Kit
This kit provides information to eligible participants to learn about your 401(k) plan features, investment funds, risk and return, and detailed description on the 14 investment options offered for your investment selection.

Catch-up Contribution Form
If you will be age 50 or older at any time during 2017, you can make up to an additional $6,000 pre-tax catch-up contribution during 2017 to your 401(k) Plan account if you meet one of the following plan limits:

  • You contribute 50% (the maximum Plan contribution rate limit) of eligible Compensation to the 401(k) Plan for the entire year or
  • You contribute $18,000 (the maximum dollar contribution limit) to
  • the 401(k) Plan in 2017.

You are not required to reach one of the above plan limits before you begin making catch-up contributions, but you must be contributing at a rate that allow you to reach one of the limits in calendar year 2017.
